Forge Health Group are looking for a new, enthusiastic nurse to extendour team. We are a friendly, supportive and motivated team who serve a diversepopulation in the north of Sheffield. The nurses play a fundamental role in thepractice, providing skills and expertise valued by the team and community.

One of the underpinning values of the practice is training anddevelopment. The nurses are encouraged to progress with training and skills tobenefit their personal development and that of the practice.

The practice works across two sites with a large staff team who arepatient centred and progressive in the way that they affect the localcommunity.

Flexibility and transport are essential requirements for the postholder.

Salary will be dependent upon experience, competence and qualifications. This maternity cover role may offer the opportunity for permanent hours as our team structure evolves.

Main duties of the job

Experience in the following is preferable but not essential:

  • Vaccination and Immunisation for adults and children including travelhealth
  • Routine procedures including ear irrigation, wound care and dressings
  • Sexual health and contraception
  • Cervical cytology
  • Chronic Disease Management to include but not limited to: Asthma,Diabetes, COPD including spirometry, CHD and LD
About us

Our surgery team consists of GP's, Nurses, Healthcare assistants, Receptionand Admin team, and Pharmacists. We are a training practiceand have GP registrars throughout the year. We also have additional teammembers who work alongside us, such as Physiotherapist, Mental Health Workers,Care coordinators and Interpreters

Job Summary

The post holder is responsible for the delivery of practice nursing services care to the practice population. Supported by the Partners, the nurse will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring and management of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Clinical practice

  • Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
  • Implement and evaluate individual treatment plans for patients with a known long-term condition.
  • Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
  • Support patients to adopt health promotion strategies that encourage patient to live healthily, and encourage principles of self-care.
  • Deliver opportunistic health promotion
  • Provide information and advice on prescribed or over-the- counter medication, on medication regimens, side effects and interactions.
  • Assess and care for patients for patients presenting with wounds, within area of expertise.
  • Support and advise women requesting information relating to family planning needs.
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
  • Promote and deliver evidence based care for patients presenting with aural conditions.

Communication

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate with and support patients who are receiving bad news.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Utilise communication skills to support patients to adhere to prescribed treatment regimens.
  • Anticipate barriers to communication and take action to improve communication.
  • Estimate and maintain effective communication with individuals and groups within the practice environment external stakeholders
  • Act as an advocate when representing the patients and colleagues viewpoints to others.

Delivering a quality service

  • Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).
  • Produce accurate and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Deliver care according to the NSF and National Institute for Clinical Excellence (NICE)

Qualifications
  • Nurse with current NMC registration
