Job Summary
Weaver Vale Practice is looking for a practice nurse, either experienced or a nurse practitioner, on a full time basis (part‑time will be considered). The role will involve working alongside the current Practice Nurse, GPs and administration staff in a friendly, well‑established teaching practice.
Main Duties
- Chronic disease monitoring in line with the QOF requirements.
- Cytology and other screening services.
- New patient medicals, urinalysis and travel clinic work.
- ECG and spirometry assessments (including peak flow and peak flow readings).
- Hypertension management and routine immunisations (including childhood immunisations).
- Chaperoning and assisting patients during examinations by another clinician.
- Requesting and reviewing pathology tests such as urine cultures and swabs.
- Contraception provision and follow‑up.
- Follow agreed clinical protocols and refer to senior nurses or GPs as appropriate.
Additional Tasks
- Act as a chaperone during examinations by other clinicians.
- Undertake home visits for chronic disease monitoring and flu immunisation.
- Actively manage and respond to blood results relevant to chronic disease monitoring.
About Us
Weaver Vale Practice is a friendly, well‑established teaching practice with 9,200 patients. We have four partners and three salaried GPs, and the practice is located in a purpose‑built health centre adjacent to Halton General Hospital in Runcorn.

Salary will be dependent on experience and skills. The post is permanent and full‑time. A standard NHS salary scheme applies.
Responsibilities
- Assist with routine patient care as directed by senior staff and GPs.
- Perform the duties listed in the Main Duties and Additional Tasks sections.
- Maintain accurate and contemporaneous records of all patient encounters in accordance with practice protocols.
- Contribute to training programmes and participate in practice meetings and audits.
Qualifications
- Regulated as a Registered General Nurse (RGN).
- Experience of working in primary care and managing chronic disease.
- Knowledge of policy and practice in areas such as cervical cytology, family planning, spirometry, travel health, sexual health and childhood immunisations.
- Experience with the Quality Outcomes Framework (QOF) is desirable.
Essential Skills
- Ability to work flexibly and share information within the team.
- Strong organisational and communication skills.
- Capacity to travel for home visits.
