Job description

This is an exciting opportunity for an enthusiastic nurse to join ourpractice team to undertake all aspects of Practice Nursing, including Long Term Condition Reviews, Baby Clinic, Dressing, Cytology, Travel Vaccine etc.

Main duties of the job

We are looking for a highly motivated nurse to provide a comprehensive, nurse led telephone triage and minor illness treatment service, along with seeing patients in their own right requesting help or guidance from the GPs and other clinical colleagues as required.

The post is a permanent position working 30 hours per week.

We are seeking an enthusiastic nurse practitioner to work within an established team. Candidates must have the following experience and skills:

Current NMC Registration

Have experience in a Primary Care setting

Familiarity with SystmOne medical system would be an advantage.

Greasbrough Medical Centre is an innovative busy, friendly practice of 3,400 patients we operate on TPP SystmOne and general office computer systems. Our practice team is committed to providing a high standard of clinical care and an excellent service to patients. We have a strong multidisciplinary team comprising of two GPs, Practice Nurse, Clinical Pharmacist, HCA and Counsellor. We also have an experienced medical support team of administrators.

Job responsibilities

The practice nurse will work as part of theprimary healthcare team providing Personal Medical Services to the patients ofpractice.

  • Assistin and perform routine tasks related to patient care as directed by seniornursing staff and GPs

  • ECGs
  • Asthma/PeakFlow Readings/Spirometry
  • ChronicDisease Management
  • Venepuncture
  • Earsyringing
  • Routineimmunisations/Childhood immunisations
  • Requestingpathology tests, for example urine culture, swabs
  • Contraception.
  • Followingagreed clinical protocols with referral to senior nurses or GPs asappropriate
  • Othertasks will include
  • Maintainingand cleaning equipment used by the nurses and GPs
  • MaintainingGP and Nurses rooms, stocking and rotating items as required
  • Chaperoningand assisting patients who are being examined by another clinician.

Health & safety

The post-holder will implement and lead on a fullrange of promotion and management their own and others health and safety andinfection control as defined in the practice Health & Safety policy, thepractice Health & Safety manual, and the practice Infection Control policyand published procedures. This will include (but will not be limited to):

  • Usingpersonal security systems within the workplace according to practiceguidelines
  • Awarenessof national standards of infection control and cleanliness and regulatory/ contractual / professional requirements, and good practice guidelines
  • Responsiblefor the correct and safe management of the specimens process, includingcollection, labelling, handling, use of correct and clean containers,storage and transport arrangements
  • Managementand maintenance of Personal Protective Equipment (PPE) for the practiceincluding provision, ordering, availability and ongoing correct usage bystaff
  • Responsiblefor hand hygiene across the practice
  • Ownershipof infection control and clinically based patient care protocols, andimplementation of those protocols across the practice
  • Activeobservation of current working practices across the practice in relationto infection control, cleanliness and related activities, ensuring thatprocedures are followed and weaknesses / training needs are identified,escalating issues as appropriate
  • Identifyingthe risks involved in work activities and undertaking such activities in away that manages those risks across clinical and patient process
  • Makingeffective use of training to update knowledge and skills, and initiate andmanage the training of others across the full range of infection controland patient processes
  • Monitoringpractice facilities and equipment in relation to infection control,ensuring that provision of hand cleansing facilities, wipes etc aresufficient to ensure a good clinical working environment. Lack offacilities to be escalated as appropriate.
  • Safemanagement of sharps procedures including training, use, storage anddisposal
  • Usingappropriate infection control procedures, maintaining work areas in atidy, clean and sterile, and safe way, free from hazards. Initiation ofremedial / corrective action where needed or escalation to responsiblemanagement
  • Activelyidentifying, reporting, and correction of health and safety hazards andinfection hazards immediately when recognised
  • Keepingown work areas and general / patient areas generally clean, sterile,identifying issues and hazards / risks in relation to other work areaswithin the business, and assuming responsibility in the maintenance ofgeneral standards of cleanliness across the business in consultation(where appropriate) with other sector managers
  • Undertakingperiodic infection control training (minimum twice annually)
  • Routinemanagement of own team / team areas, and maintenance of work spacestandards
  • Wastemanagement including collection, handling, segregation, containermanagement, storage and collection
  • Spillagecontrol procedures, management and training
  • Decontaminationcontrol procedures, management and training, and equipment maintenance
  • Maintenanceof sterile environments
  • Demonstrate due regard for safeguarding and promoting thewelfare of children.

Qualifications
  • Nursing & Midwifery NMC Registered
  • Diploma in COPD, Asthma, Diabetes, Spirometry- long term conditions
  • Qualified to undertake Cytology
Experience
  • Experience of working as a Practice Nurse
  • Previously worked in a General Practice setting undertaking long terms conditions i.e Diabetes, COPD, Asthma etc
