We are a medium sized friendly practice. Our nursing team all work together as a team and help each other, and also have good interactions with all the other teams in the practice. We are looking for a team player to join our friendly team. We are also a training practice with Registrars, student nurses and medical students.

Main duties of the job

Working closely with other clinicians to provide basic and higher level skill to ensure patient receive the best care possible. To ensure QOF coding is correct and reviews are followed up

We have 7800 patients and our team consists of 2 GP Partners, 4 Salaried GP's 3 Nurse Prescribers, 2 Nursing Associates, 1 Phlebotomist and a Physician Associate. We also have 1 Practice Manager, 1 Assistant Practice Manager, 1 Care Co-Ordinator, 2 Secretaries and 7 receptionist.

JobPurpose: This position in the practice aims to provideprovision of high quality nursing care to meet the needs of the practicepopulation; which includes treatment, screening, preventative care, patienteducation, clinical quality systems maintenance; and the supervision of anyless qualified clinical colleagues, while working in partnership with otherclinical, management and administrative colleagues to meet stated practicedevelopmental goals.

Keyresponsibilities:

Clinical:

It is accepted that all clinical skillsand competencies may not be available on joining. Training will be available ifrequired and agreed.

Toprovide basic and higher levels of nursing care as needed for patients.

Toprovide care for minor injuries, dressings, ear car (including dressings)including pressure bandaging and higher level wound care, liaising directly,where appropriate with community and secondary care counter-parts, escalatingto a GP as necessary and also provide supervision for less qualified nursingcolleagues in the practice.

Tosupport the annual flu campaign and other mass population vaccinationprogrammes as needed in line with national guidance.

Toconduct family planning consultations and assist the GPs in providing LARCservices. T

Toact as a chaperone or similar support for other colleagues as needed.

Toconduct anthropometric measurements, blood pressure, spirometry, urinalysis,phlebotomy, microbiology samples, ECG, ABPM, peak flow and other practice basedtests as needed in the course of the care of patients.

Toprovide patient health promotion/ disease prevention education and counsellingfocusing on health behaviour change.

Toassist members of the clinical team in providing a seamless service through anillness episode providing specific nursing capabilities and directing thenursing component of any illness episode.

Toliaise with community nursing colleagues to continue the provision of nursingcare in the home if patients are unable to attend the practice and to transferor share care as appropriate, visiting outside the practice if needed onexceptional circumstances to meet the needs of patients.

Toprovide support to the GPs in the management of chronic diseases such asdiabetes, COPD, Asthma, Hypertension etc. by providing specifically definedclinical sessions, working closely with the GP to manage a patients overallcare.

Tomanage and maintain chronic disease registers and work with admin staff toconstruct and maintain robust call and recall systems for chronic disease,cervical cytology, mammography, immunisation and other populations based healthpromotion and disease prevention care programmes, escalating where necessarypatients who need intervention from a GP to insure a review as appropriate,constructing and maintain care pathways where patients receive appropriateinvestigations and reviews by an appropriate clinician at the appropriate timeinterval.

Topromote the health and well being of patients by counselling patients on thenature of their conditions, the use of their medications, diet, exercise andother health behaviours.

Toprovide support and guidance to patients in times of crisis, breaking bad newswhere necessary to patients and carers and provide on going support as needed.

Tocounsel patients on the meaning of test results and other medical information,conducting phone and in person consultations; triaging patients to anappropriate clinician, finishing a line of investigation, or arrange additionalinvestigations as appropriate in line with practice guidelines and protocolsand in partnership with clinical colleagues.

Toeventually establish and participate in a robust system of clinical audit inpartnership with other clinical and administrative colleagues, leading in areasappropriate, such as infection control, to a nursing skill set and supportingin other areas such as medicines management.

Tomaintain a register of patients receiving nursing on going nursing care.

Toassist in the identification of patients at risk of acute decompensation oftheir baseline state of health and participate in measures needed to avoidemergency admissions and acute unplanned activity.

Toprovide leadership and support in all practice services, PMS, LES, DES andother incentive schemes.

Manage and treatlong-term conditions, in line with national and local policies and practiceneeds

Administrative:

Tomaintain accurate patient records, entering onto the practice system theappropriate agreed codes, using agree standards of data entry, templates andprotocols, completing all documentation, computerised and paper-based, as isrequired for good patient care.

Tomaintain and document in robust systems of data collection, audit and review ofall clinical activities, protocols, care pathways and clinical systems,liaising with fellow colleagues in the practice and in the local and nationalhealth system as needed to insure good patient care and service is maintainedand to satisfy the standards of external regulatory bodies such as the CQC etc.

Tocollate the necessary data collection for QOF and other incentive schemes asneeded by the practice, prompting other members of the team as needed forassistance in gathering the required data.

Tocreate and maintain in cooperation with other nursing and admin colleagues asystem of clinical stock and equipment maintenance, calibration, and supply;working closely with the practice manager to insure cost efficient purchasingof perishable and expensive items.

Toensure that all statutory requirements are met for a safe and therapeuticenvironment for patients and staff in cooperation with the practice manager andpractice services manager.

Tomaintain all documentation needed to carry out your clinical duties.

Toparticipate in Infection Control procedure and protocols and theirimplementation within the practice.

General:

Tomaintain your own annual registration with the NMC and adhere to its Code ofProfessional Conduct.

Topromote a professional approach at all times by dress and attitude to patientsand staff, both in and out of the Practice, setting an example for those underyour direct authority and to the team as a whole.

Tomaintain you own continuous CPD and insure a PDP is kept up to date in linewith the practice and nursing team development plans.

Toparticipate in, and attend practice meetings and in-house training.

Provideappropriate constructive feedback to all members of the team and at times beingprepared to challenge your colleagues, clinical and administrative, to maintainhigh standards of professionalism, patient care, service and excellence,prompting others to fulfil their own explicit and implicit responsibilities topatients and colleagues.
