Christchurch Medical Practice in Poole seeks a motivated Practice Nurse to join our friendly nursing team. You will deliver high-quality, patient-centred care within a supportive multidisciplinary environment, supporting health promotion, disease prevention and chronic disease management.

You'll work alongside GPs and healthcare assistants, participate in quality improvement and ensure accurate record-keeping in line with NMC standards.

Qualifications

  • Registered Nurse with current NMC registration and active PIN.
  • Understanding of audit and clinical governance.

  • Experience in primary care settings is desirable.

Responsibilities

  • Delivering patient-centred primary care including assessment, screening and health promotion.
  • Vaccinations and injections following national guidance.
  • Venepuncture, ECGs, and other diagnostic procedures within scope of practice.
  • Support long-term condition management (asthma, COPD, diabetes).
  • Wound care and dressings; assist with minor surgery where needed.
  • Maintaining accurate clinical records in line with NMC standards.

Job summary

We are seeking a motivated and compassionate Practice Nurse to join our friendly and supportive nursing team at Christchurch Medical Practice.


The successful candidate will play an important role in delivering high-quality nursing care to our patient population, supporting health promotion, disease prevention, screening programmes and long-term condition management. Working alongside GPs and the wider multidisciplinary team, you will provide evidence-based care, contribute to quality improvement initiatives and support the delivery of excellent patient outcomes.


This is an exciting opportunity for an experienced nurse looking to develop their skills within a progressive primary care environment.



Main duties of the job


  • Delivering assessment, screening, treatment services and health promotion advice.

  • Undertaking vaccinations, immunisations and injections in accordance with national guidance and local policy.

  • Performing venepuncture, specimen collection, ECGs, ambulatory blood pressure monitoring, FeNo and other diagnostic procedures within scope of practice.

  • Supporting patients with long-term conditions such as asthma, COPD and or diabetes.

  • Providing wound care, leg ulcer management and compression therapy following appropriate training and competency assessment.

  • Undertaking cervical screening and supporting sexual health and contraception services where competent to do so.

  • Assisting GPs with minor surgery and other clinical procedures.

  • Promoting self-care and providing lifestyle advice, including smoking cessation and health improvement interventions.

  • Maintaining accurate clinical records and ensuring compliance with NMC standards, practice policies and national guidelines.

  • Supporting infection prevention and control, safeguarding responsibilities and health and safety requirements.

  • Contributing to Quality and Outcomes Framework (QOF) delivery and service development initiatives.

Qualifications


Essential


  • Registered Nurse with current NMC registration and active PIN.

  • Evidence of appropriate professional knowledge and skills.

  • Understanding of audit and clinical governance.

  • Understanding of health and safety requirements.



Desirable


  • Chronic disease management qualifications.

  • Competency and relevant certification in Asthma, COPD and Diabetes reviews.

  • Basic or Advanced Life Support certification.

  • Mentorship qualification.

  • Experience of treatment room nursing.

  • Phlebotomy and venepuncture.

  • ECG recording.

  • Vaccinations and immunisations.

  • Wound care and dressings.

  • Assisting with minor surgery.

  • Health screening and preventative care.

  • Long-term condition management.

  • Experience supporting patients and carers.



Desirable


  • Experience working within primary care/general practice.

  • Experience of GMS contract and QOF requirements.

  • Experience in health improvement, public health or community-based care
