Job description

Exciting opportunity for an experienced Practice Nurse to join our friendly Plymouth PCN.

We are an innovative 5‑surgery PCN serving a population of 34,000. We encourage training and education for our staff and support student nurses, paramedics, physiotherapists and GP trainees.

Main duties of the job
  • Health promotion, chronic disease management, and health prevention.
  • Support the management team in reviewing clinical policies and procedures.
  • Initiate and maintain health screening and promotion clinics as needed.
  • Manage nursing care for patients with chronic disease, using recall systems and practice computer systems.
  • Provide specialist care for registered patients in Diabetes, Cervical Smears, Contraception, Baby immunisations, and Adult and Child vaccinations.
  • Provide routine treatment in general clinics, including wound management, blood tests, and ECGs.
Qualifications
  • UK Registered nurse with NMC.
  • Minimum 2 years post registration.
  • Special interest in diabetes at level 6 or 7.
  • Experience working in a General Practice setting.
  • Knowledge of cervical cytology and immunisations.
  • Experience in wound care and long‑term conditions.
  • Understanding of audit processes.
