A regional healthcare provider in Oakham is seeking a Practice Nurse to deliver evidence-based nursing care and manage chronic conditions in patients. The successful candidate will work collaboratively within a clinical team and provide leadership to the nursing team. The Practice Nurse worksas part of the clinical team responsible for the delivery of general practicenursing to the Empingham Medical Centre practice population. Key elements ofthe role are delivering evidence-based practice for patients presenting withlong term conditions and the provision of preventative health care. An autonomous practitioner the nurse isresponsible for the care delivered, demonstrating critical thinking skills inclinical decision-making in the management of patients. He/she also workscollaboratively within the general practice team to meet the needs of patientsand support practice objectives, policies and procedures. He/she will act assupport to the nurse manager by providing leadership within the practice nurseteam as well as making practice nurse team decisions in the absence of thenurse manager.
Manage patients in achronic disease management clinic with diabetes mellitus, stable hypertension,asthma / COPD and deliver key performance indicators
Provide telephoneadvice to patients when required
Proactively identify,diagnose and manage treatment plans for patients at risk of developinglong-term conditions
Adult and childhoodimmunisation including seasonal campaigns
Cervical Screening
Coil removals and coilchecks
Contraception
Removal of sutures
Other vaccinationsincluding travel
Empingham Medical Centre is situated in the village of Empingham, on the edge of Rutland Water in the heart of the picturesque County of Rutland. We provide GP and Nurse appointments and we are a Dispensing Practice.
The Practice area covers over 120 square miles in the villages around Rutland Water. We are members of the East Leicestershire and Rutland Integrated Care Board (ELRICB) and mainly refer for secondary care to Peterborough City Hospital and its clinics at Stamford. A smaller number of referrals are made to University Hospitals Leicester as well as the local community based facilities.
We are a very popular Practice, rated very highly in local and national patient satisfaction surveys. We were rated GOOD in all areas at our CQC inspection which took place in April 2025.
The Practice is part of the Jubilee Academy and is actively involved in teaching 3rd Year Medical Students from Leicester Medical School.
