Job responsibilities

To provide highquality care based on the needs of the practice population which includestreatment, screening, preventative care and patient education

To assess, plan,deliver and evaluate care holistically

To deliver care inaccordance with agreed practice protocols and guidelines

Assist indeveloping new services and complementing and extending the range of servicesoffered to patients

RESPONSIBILITIES

Key areas ofresponsibility will include:

Provide treatmentand care in conjunction with the GPs or independently as appropriate andaccording to practice policy and guidelines

Take responsibilityfor own clinics and time management

Review and filingof own test results and those relating to chronic disease

Advise patients ongeneral health and minor ailments with referral to the GP as necessary

Immunisations andvaccinations including childhood immunisations

Holidayvaccinations and travel advice, inline with GP Contract.

Venepuncture

First Aid forpresenting emergencies

Diagnostic testssuch as ECG, urinalysis, swabs, blood tests

Responsibility forthe nursing management of patients with chronic disease/long term conditionsincluding diabetes, CHD, CVA, asthma, COPD, CKD, mental health, learningdisabilities, autism and hypertension.

To initiate,develop and implement protocols in relation to patients with chronic disease

To carry outcervical cytology

Carry out newpatient health checks, well man and well woman checks

Actively promotehealth education and weight management advice

Provide familyplanning and contraception advice

Act as chaperonewhere required

To promote and runhealth campaigns as appropriate

Ensure the stockand immunisation stocks are monitored and kept up to date

To carry out riskassessments in accordance with COSHH regulations

Keep up to datewith practice policies and procedures, especially in relation to recordingsystems, administration of drugs, child protection, confidentiality, health andsafety

Adhere to infectioncontrol procedures and ensure practice policies are up to date

Cleaning ofequipment and disposal of waste

To be competent inthe use of the computer ensuring patients details are correctly input inaccordance with practice guidelines

To activelycontribute and oversee QOF targets directly relating to the practice nurse role

Support PCNinitiatives when required

RELATIONSHIP

Responsible to thePartners for clinical issues and the Practice Manager for administrative issues

To work closelywith other members of the team, practising good teamwork at all times

Liaise with otherteam members e.g. admin and reception staff, District Nurses, Health Visitors,Community Matrons, School Nurses etc.

Liaise withsecondary care and social services to ensure safeguarding, quality andcontinuity of care

Attend monthlymulti-disciplinary meetings in-line with clinical responsibility.

ACCOUNTABILITY

Maintain own annualregistration with the Nursing and Midwifery Council and work within the scopeof the NMC guidelines

Be personallyaccountable for own practice and promote a professional approach to patientsand staff

Acknowledge anylimitation in knowledge and competence and practice within own limits at alltimes, highlighting any training needs to the Practice Manager

Maintain accuraterecords in line with NMC guidelines including data collection, completion oftemplates and read coding

Maintain owncontinuous professional development and ensure re-validation and PDP is kept upto date in line with the requirements of the practice and the NMC

Maintain own carinsurance ensuring cover is in place for work related activities

PRACTICEDEVELOPMENT

Attend andcontribute to meetings both internally and externally as required

Participate inPractice Development and planning

Attend courses andtraining as identified at appraisal and agreed with the Practice Manager

Co-operate/participatewith research within the practice

PREMISES

Be aware and notifythe Practice Manager of any Health and Safety and hazardous conditions

To be familiar withprocedures if the fire alarm is raised and assist patients out of the practicewithout putting own life at risk

Ensure safetyprocedures are carried out in order to maintain a safe environment foremployees, patients and visitors.
