Job description

We are looking for a full time Nurse to complement our excellent existing nursing team.

We are a very friendly practice who are dedicated to our patients and their care.

Main duties of the job

To be responsible for the delivery of practice nursing services, working as part of the practice team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management and health prevention.

We are a very friendly Practice .We are a 6 Partner Practice with PP's, ACP's, Practice Nurses, Nursing Associate's, HCA's and a Phlebotomist.Our Clinical team is supported by our Reception and Administrative teams.

Job responsibilities

Todevelop, implement and embed health promotion and well-being programmes

Toimplement and evaluate individual treatment plans for chronic disease patients

Toidentify, manage and support patients at risk of developing long termconditions, preventing adverse effects to patients health

Toprovide routine nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF

Toprovide wound care (ulcer/Doppler etc.) to patients

Torequest pathology services as necessary

Toprocess pathology results as required

Toprovide chronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required

Tomaintain accurate clinical records in conjunction with extant legislation

Toensure read codes are used effectively

Tomaintain chronic disease registers

Todevelop, implement and embed well woman clinics

Todevelop, implement and embed well man clinics

Tochaperone patients where necessary

Toprioritise health issues and intervene appropriately

Tosupport the team in dealing with clinical emergencies

Torecognise, assess and refer patients presenting with mental health needs

Toimplement vaccination programmes for adults and children

Tosupport patients in the use of their prescribed medicines or over the countermedicines (within own scope of practice)

Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

Todelegate clinical responsibilities appropriately (ensuring safe practice andthe task is within the scope of practice of the individual)

Tosupport the clinical team with all safeguarding matters in accordance withlocal and national policies

Tounderstand practice and local policies for substance abuse and addictivebehaviour, referring patients appropriately

To deliveropportunistic health promotion where appropriate
