A healthcare provider in Manchester is seeking a part-time Practice Nurse for a two-year fixed term contract. The successful candidate will provide high-quality nursing services to patients, manage chronic diseases, and work collaboratively with a dedicated multidisciplinary team. We are looking for a Practice Nurse for 20 hours per week on a fixed term two year contract.

The post holder will work collaboratively with the multi-disciplinary team to meet the needs of the practice and patients, and will play an integral part in delivering the KPIs for the Practice.
You will have a varied caseload, including: Chronic Disease Management, preventative care, immunisations, patient education.

The post holder will be responsible for leading on the practice Infection Prevention and Control audits.
