Holland House Surgery has an exciting opportunity for aPractice Nurse to join our enthusiastic and friendly team. We are looking for someone who will work aspart of the nursing team providing high level nursing care to our 11000+ patients.

We are a Gold award winning training practice for both GPsand students and offer a supportive learning environment and will providerelevant training opportunities for the right candidate. Main duties of the job

We are seeking applicant who are adaptable, highly motivated, organised and work well within a team. Experience in chronic disease management, particularly Diabetes and Respiratory , COPD, cervical screening and childhood immunisations are desirable. However, as mentioned, we are willing to fully train a committed and enthusiastic nurse in these areas if you are looking for a career change or are new to nursing.

Job responsibilities

We are looking for a confident individual that can join our supportive and forward-thinking practice. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

You will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients and to support the delivery of their carewithin your scope of practice to the entitled patient population

The core responsibilities are:

Managing patients presenting with a range of acute and chronic medical conditions both in practice and at home (if required).

Clinical responsibilities:

In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home (if required), and dealing with queries, paperwork and correspondence in a timely fashion, completing contractual requirements for patients

  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Screening patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current practice disease management protocols, developing care plans for health
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Implement vaccination programmes for adults and children
  • Other responsibilities within the organisation
  • To ensure that any in house audit and requirements are achieved
  • To record all clinical activity and health promotion data in the Practice computer system
  • Awareness of and compliance with all relevant practice policies/guidelines, e.g. confidentiality, data protection, health and safety
  • Contributing to evaluation/audit and clinical standard setting within the organisation
  • Contributing to the development of computer-based patient records
  • Contributing to the summarising of patient records and Snomed-coding patient data
  • To maintain full accurate and contemporaneous records of activities both written and computerised as applicable to an agreed standard. The practice has a paperless approach and as such the computer records form the main record of all interactions with patients
  • Attending training and events organised by the practice or other agencies, where appropriate.
  • To work with multi-disciplinary team within the practice, and across the wider ICB to promote integrated and seamless pathways of care
  • To maintain good lines of communication with the partners, practice management team and wider multi-disciplinary and administrative team
  • To promptly notify the Practice Manager or deputy of any absence
  • To observe Health and Safety rules as outlined in the Practice Health and Safety Policy
  • To maintain standards of conduct and dress to maintain public confidence
