Job description

An exciting opportunity has arisen within the practice for a practice nurse to join our friendly and supportive team. We are a forward thinking practice who have a patients centered care approach, serving 10500 patients. Candidates should be willing to work in an involving role be enthusiastic, empathetic and have a flexible approach. Excellent communication skills are essential to this role.

Main duties of the job

Knowledge of needs of patients with long-term conditions

Cervical cytology, immunisation and vaccination skills

Spirometry monitoring

Change management skills and ability to support patients to change lifestyle

Excellent communication skills, both written and verbal

Ability to communicate difficult messages to patients and families

Aware of accountability of own role and other roles in a nurse led service

Knowledge of health promotion strategies

Awareness of clinical governance issues in primary care

Knowledge of patient group direction

Experience of TPP S1

Membership of a professional body

Motivated and enthusiastic

Willing to learn new skills

Flexible

Adaptable to and embracing of change

Experience of working in a Primary Care environment is preferred.

Kingston Health is a friendly practice of 10500 patients based over 2 sites covering both west and east of the city.

We have 4 GP partners, 1 Physician Associate. 1 Pharmacist , 2 Practice Nurses, 2 Health Care Assistants supported by ourknowledgeable administrative team. We are also a training Practice with F2s.

Job responsibilities

Duties andResponsibilities

Clinical

Tomanage patients with chronic disease, running specific clinic sessions andmaintaining robust call and recall systems

Participatein national screening work such as cervical cytology and deliver the nationalimmunisation and vaccination schedule

Providetravel health advice and administer appropriate vaccinations

Providedirectly and assist GPs with contraceptive services

Interactwith patients/parents/carers and provide appropriate ongoing support to meetidentified needs

Undertakehealth promotion to help educate patients

Select,carry out and interpret results of specific tests if required

Tomaintain accurate patient records and enter onto the computer using Ardens templates

Ensureaccurate completion of all necessary documentation associated with patienthealth care

Tocollect data and participate in regular Practice audit and research projects

Tocontribute to the preparation of any Practice development plans

Tomaintain own annual registration with the NMC and adhere to Code ofProfessional Conduct

Tomaintain a safe and therapeutic environment for all nursing team staff,patients, carers and co-workers from other agencies. Ensure all statutoryregulations (e.g. Health & Safety, Infection Control etc) are implemented andadhered to

Topromote a professional approach at all times by attire and attitude topatients and staff both in and out of the Practice

Tomaintain own continuous professional development and ensure PDP keptup-to-date in line with the requirements of the Practice

Attendand participate in staff and Practice meetings and in-house and external clinicaltraining sessions

Anyother reasonable duties that may be required from time-to-time
