• Manage chronic diseases like Asthma and Diabetes.
• Perform wound care and cervical screening.
• Support the clinical team and service development.

Job description

Responsibilities

• Chronic disease management (Asthma, COPD, Diabetes, CHD etc.)
• Wound care and management
• Cervical screening
• Child and adult immunisations
• Ear irrigation
• Blood pressure monitoring and health checks
• Requesting and interpreting pathology tests
• Spirometry and peak flow readings
• Supporting the wider clinical team and contributing to service development

Were looking for an experienced and motivated Practice Nurse to join our supportive and well‑established nursing team for 27 hours a week Tuesday, Wednesday and Friday.
