The Coastal Partnership is seeking a capable and highly motivated Practice Nurse to join our team. We are looking for a nurse with enthusiasm, commitment, and good communication skills to join our loyal and well-established nursing and multi-disciplinary team delivering care.
To provide and maintain a high standard of nursing care for patients working alongside the healthcare team. Support and promote the aims and ethos of the practice.
Management of chronic diseases from diagnosis, monitoring and development of individual care plans, agreeing where appropriate with the patient and other health care professionals.
Experience in childhood immunisations and cervical screening desirable.
