About the Practice

A forward-thinking and patient-centred GP practice in the NG6 area is seeking a dedicated Practice Nurse to join its friendly multidisciplinary team. The practice focuses on delivering accessible, high-quality primary care and improving long-term health outcomes for the local community.

You’ll work alongside GPs, clinical pharmacists, healthcare assistants, and administrative colleagues within a supportive and well-organised environment that encourages professional development and continuous improvement.

Role Summary

As a Practice Nurse, you will play a vital role in the delivery of holistic patient care. You will be responsible for providing high-quality nursing services, chronic disease management, health promotion, and screening activities. The role offers autonomy, clinical responsibility, and opportunities for personal and professional growth.

Key Responsibilities
  • Deliver nurse-led clinics for chronic conditions such as diabetes, COPD, asthma, hypertension, and cardiovascular disease
  • Conduct cervical cytology screening and child/adult immunisations (including travel and flu)
  • Undertake wound care, ear irrigation, ECGs, and other treatment room duties
  • Contribute to the management of patients with long-term conditions in line with QOF and local protocols
  • Support and participate in health promotion, NHS Health Checks, and lifestyle education initiatives
  • Maintain accurate clinical documentation and contribute to audit and quality improvement activities
  • Work collaboratively within a multidisciplinary team to deliver safe and effective patient care
  • Uphold infection control, safeguarding, and CQC standards
  • Participate in ongoing professional development, appraisals, and mandatory training
Essential Criteria
  • Registered Nurse (NMC) with valid UK registration
  • Experience working in a primary care or community setting
  • Competent in cervical cytology and routine immunisations
  • Knowledge of chronic disease management and evidence-based practice
  • Excellent communication and interpersonal skills
  • Ability to work both independently and as part of a team
  • Commitment to high standards of clinical care and patient experience
Desirable Criteria
  • Independent Nurse Prescriber qualification (or working towards it)
  • Experience in spirometry, ECGs, wound management, or minor illness
  • Previous involvement in quality improvement, service development, or mentorship
  • Understanding of clinical systems such as EMIS Web or SystmOne
