A community-focused GP practice in Coventry is seeking a skilled Practice Nurse to manage patients with long-term conditions. You will deliver clinical services including immunisations, chronic disease management, and health promotion in a supportive, multidisciplinary environment. Ideal candidates will have a strong background in primary care nursing, experience in chronic disease management, and relevant qualifications. This role offers opportunities to enhance patient care and collaborate with a dedicated healthcare team.
Tobe responsible for the implementation of processes for the effective managementof patients with long-term conditions using evidence-based practice includingcare for elderly and housebound patients.
Workingas a senior member of the practice multidisciplinary team, the post holder willensure nursing services are delivered effectively to the entitled patientpopulation.
ThePractice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, healthpromotion, well women and well man clinics, as well as actively supporting thepractice management team in the reviewing and delivery of clinical policy andprocedure.
Phoenix Family Care Surgery must ensure that thepost holder has access to appropriate clinical supervision and an appropriatenamed individual in the organisation to provide general advice and support on aday-to-day basis.
The role is varied and rewarding, including long-term condition management, immunisations and vaccinations, cervical cytology, wound care, health promotion and supporting our QOF and enhanced services.

Primary keyresponsibilities
The following are the core responsibilities of thePractice Nurse. There may be, on occasion, a requirement to carry out othertasks. This will be dependent upon factors such as workload and staffinglevels:
a.To support the Infection Prevention Control(IPC) lead and to work to the IPC Policy supporting the implementation ofaudits, training, support and evidence of compliance to the practice manager
b.To support the production of the IPC annualstatement in conjunction with the practice manager
c.To develop, implement and embed an effectivecare management programme for the frail and housebound patients registered atthe practice
d.To assess the needs of patients ensuring theprovision of healthcare is appropriate, incorporating evidence-based practice
e.To develop, implement and embed healthpromotion and wellbeing programmes
f.To implement and evaluate individual treatmentplans for chronic disease patients that promote health and wellbeing
g.To identify, manage and support patients atrisk of developing long-term conditions, preventing adverse effects topatients health
h.As required, to provide routine nursing careto patients in accordance with clinical based evidence, NICE and the NSF
i.As required, to diagnose and manage acute andchronic conditions
j.To prescribe medication for patients inaccordance with PGDs and own scope of practice
k.To review patient medications to enhancecompliance
l.To provide guidance to patients in the use ofprescribed and over-the-counter medications regarding side effects anddosages
m.To co-ordinate the provision of travelmedicine services
n.To request pathology services and processpathology results as required
o.To oversee the provision of chronic diseaseclinics, delivering patient care as necessary, referring patients tosecondary/specialist care as required
p.To maintain accurate clinical records inconjunction with extant legislation
q.To ensure read codes are used effectively byall members of the nursing team
r.To ensure the appropriate maintenance and useof chronic disease registers
s.To develop, implement, embed and participatein well woman clinics
t.To develop, implement, embed and participatein well man clinics
u.To ensure compliance with the practicechaperone policy
v.To assist GPs with minor surgery when required
w.To prioritise health issues and interveneappropriately
x.To support the team in dealing with clinicalemergencies
y.To recognise, assess and refer patientspresenting with mental health needs
z.To implement vaccination programmes for adultsand children
aa.To support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice)
bb.To liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients etc.)
cc.To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)
dd.To support the clinical team with allsafeguarding matters, in accordance with local and national policies
ee.To understand practice and local policies forsubstance abuse and addictive behaviour, referring patients appropriately
ff.To deliver opportunistic health promotionwhere appropriate
gg.Regularly monitor and check medical equipmentand report faults to the practice manager.
Secondary responsibilities
In addition tothe primary responsibilities, the Practice Nurse may be requested to:
a. Develop and reviewaudit protocols and process for the practice, effectively utilising the auditcycle
b. Develop, review andimplement nursing protocols in conjunction with the partners
c. Support all membersof the nursing team, providing guidance when necessary, conducting appraisalsand training needs analyses as required
d. Participate inlocal initiatives to enhance service delivery and patient care
e. Support andparticipate in shared learning within the practice in order to improvepatient care
f. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate
g. Participate in thereview of significant and near-miss events applying a structured approach,i.e., root cause analysis (RCA)
h. Coordinate thenursing team rota, ensuring sufficient staff are available to meet patientneeds
i. Lead the planningand implementation of changes within the nursing team, enhancing theprovision of services to patients
j. Support thepractice team with the reviewing and implementation of practice policies andprotocols, ensuring conformance to extant legislation
