We prefer that you are able to drive as there will be home visiting required for some clinics
Thepost holder will be expected to liaise appropriately with all members of themultidisciplinary team to provide an expert service to service areas related tothe speciality.
To be responsible for allaspects of which involves assessment, planning, delivery and evaluation ofdirect patient care and function as an independent practitioner/clinical expertby virtue of their in-depth knowledge and experience.
Tosupport clinicians in the multidisciplinary team in providing clinical andmanagerial expertise in all aspects of the speciality ensuring the higheststandard of nursing care
To lead, initiate and manageprojects that will deliver enhanced patient care and inform best practicestatements.
Tomonitor and ensure a positive focused service incorporating patients' feedbackto improve patient care.
To act as aneffective role model and mentor promoting excellence through practice.

To undertake clinics providingexpert advice, counselling and individualised patient focused treatment and care.
To accept & prioritisereferrals & autonomously manage a specific caseload without directsupervision, using own judgment & discretion.

To undertake all relevant procedures in accordance with WFFederated GP Network policies and ensure that these procedures and policies areunderstood and adhered by all staff, within own management responsibility.

MAINDUTIES AND RESPONSIBILITIES
CLINICALRESPONSIBILITIES
You must be an independent Nurse Prescriber and willing to complete some home visits
To accept & prioritisereferrals & autonomously manage a specific caseload without directsupervision, using own judgment & discretion.
Undertake such clinicalprocedures as necessary in the - management of individualised patient care,demonstrating a high standard of nursing practice.
To actively participate in thedevelopment of relevant evidence based clinical guidelines/policies relating tothe speciality.
To establish effective workingrelations with all members of the multi-disciplinary team topromote good communication and a multi-disciplinary approach to patient care.
To ensure accurate andcomprehensive consultation records are maintained using appropriate written andelectronic documentation.
To collect data and provideessential information in a timely and accurate manner when requested for e.g.audits or monitoring CQC standards.
To ensure that all clinicaland non-clinical incidents, including serious incidents are properly reported,- investigated and changes made to avoid recurrence.
To give expert evidence basedprofessional advice to healthcare professionals in primary and secondary patientsand relatives, other clinical and non-clinical staff of WF Federated GPNetwork
Act in accordance with TheCode - professional standards of behaviour for nurses and midwives (Nursing andMidwifery Council (NMC) 2015)
RESPONSIBILITY FOR PATIENTS
To respond to highlycomplex/demanding patient situations.
To lead clinically andprofessionally, making autonomous decisions in clinics
To attend Home Visits whererequired.
To assess and diagnosespecific symptoms within the scope of their practice, advise medical andnursing staff and /or prescribe medications according to patient's requirements,writing a record in the medical notes and reassessing according to theindividual patient needs.
Where appropriate referpatients, who need to see a Haematologist to the Anticoagulation clinic as per clinical protocol.
To ensure communication withpatients and significant others are established, ensuring empathy andreassurance regarding, assessment, care and treatment and to ensure informedconsent is obtained from patients.
Be accountable at all timesfor ensuring that safeguards are in place for patients, in particular those adultsat risk.
To respect patient'sconfidentiality, ethnic cultural and spiritual needs.
Provide relevant, highquality, up to date written and verbal patient information regarding venousAnticoagulation ensuring the patient/carers have an adequate understanding
Monitor Anticoagulant therapy,both oral, and where appropriate, injectable Anticoagulant therapy, as perguidelines and protocols.
Advise on duration of Anticoagulationas per protocol and follow appropriate patients in anticoagulation specialist- ledclinic.
Provide expert advice onprevention of venous Anticoagulation in future.
Observation and recording of vital signs
To ensure that all patientsare cared a clean and well-maintained clinical area, ensuring that-all staff areaware that they are directly responsible for cleanliness standards when incharge and escalating issues when necessary.
RESPONSIBILITYFOR LEADING AND MANAGING
To liaise, if required, withother hospital-staff and departments to promote and maintain effective workingrelationships, ensuring high standards of care and service.
To act as an appropriate andeffective role model at all times.
To act as an effective mentor,preceptor and support other staff in performing this role.
To act as a resource,supporting and motivating staff members.
To participate in trainingneeds analysis for own clinical area.
To actively participate in ownappraisal/personal development plan with the WF Federated GP Network appraiser.
RESPONSIBILITY FOR POLICY AND SERVICEDEVELOPMENT
Resolve any concerns aboutlevels of care or incidents involving patients escalating to the lead nurse andreporting via the trust's incident reporting system.
RESPONSIBILITYFOR FINANCIAL AND PHYSICAL RESOURCES WHEN ON DUTY
Accountable for generalclinical environment-needs, being alert to possible sources of cross-infection, maintaining safe and clearwalkways and patient areas, ensure fire exits remain clear and equipment andsupplies are stored correctly.
Accountable for the safecustody, storage, usage and; recording of all medication includingcontrolled drugs in accordance with WF FedNet policy.
Accountable for efficient andeconomical use of department supplies.
RESPONSIBILITYFOR INFORMATION RESOURCES
To be responsible for ensuringconfidentiality and safekeeping of patient records.
RESPONSIBILITY FOR RESEARCH, EDUCATION& DEVELOPMENT
To actively participate in thedevelopment of relevant evidence based clinical guidelines/policies relating tospeciality.
To be aware of the researchprocess and current trends.
In collaboration with the clinicallead, audit agreed clinical guidelines of practice, policies and benchmarkswithin area of clinical responsibility.
Through the appraisal system,ensures own training needs are identified and mandatory training is kept up todate.
To be responsible fordeveloping and sustaining own knowledge, clinical skills and professionalawareness.
GENERALRESPONSIBILITIES
Infection Control
Infection control is everyone's responsibility. All staff,both clinical and non-clinical, are required to adhere to the WF Federated GPNetwork Infection Prevention and Control policies and procedures and The Health and Social Care Act 2008: Codeof Practice for the prevention and control of healthcare associated infectionsand make every effort to maintain high standard of infection control at alltimes thereby reducing the risk of Healthcare Associated infections.
It is the duty of every member of staff to take personalresponsibility for the prevention and control of infection, as laid down in theWF Federated GP Network policies and procedures which reflect the statutory requirements of the Hygiene Code.
To ensure that monitoring ofclinical practice is undertaken at the agreed frequency.
To ensure that the clinicenvironments are clean and maintained to the highest standards; ensuring thatshortfalls are rectified or elevate as necessary.
To ensure that all relevantmonitoring data and issues are provided to the Directorate's Governancestructures.
