Job description

Eltham Medical Practice is seeking an experienced and motivated Practice Nurse to join our friendly and supportive primary care team.This is an excellent opportunity for a nurse who is passionate about delivering high-quality, patient-centred care in a dynamic general practice setting.

Main duties of the job

Deliver a full range of nursing services to our patient population, including immunisations, cervical cytology, wound care, long-term condition management, and health promotion.

Provide evidence-based care in accordance with practice protocols and national guidance.

Support chronic disease management clinics (e.g. diabetes, asthma, COPD, hypertension).

Undertake and interpret investigations such as ECGs, blood pressure monitoring, and spirometry (if trained).

Maintain accurate clinical records using EMIS Web.

Contribute to QOF, IIF, and other quality targets within the practice.

Participate in multidisciplinary team meetings, audits, and service development.

Job responsibilities

Key responsibilities:

Provide nursing care to patients of all ages, including wound management, cervical screening, immunisations, and health checks.

Support the management of long-term conditions through review clinics and patient education.

Administer and monitor vaccinations and injections in line with national guidelines.

Provide lifestyle advice and promote self-care and health improvement.

Participate in infection control, clinical audits, and cold chain maintenance.

Work collaboratively with the multidisciplinary team to deliver coordinated care.

Maintain professional standards and comply with NMC revalidation requirements.

Contribute to training and continuous improvement within the practice.

Person Specification
Experience
  • Previous experience in a primary care or community nursing role
  • Competence in chronic disease management (e.g. diabetes, asthma, contraception, COPD, hypertension)
  • Proficient in wound care, immunisations, and cervical cytology (if trained)
  • Understanding of infection control, cold chain management, and safeguarding principles
  • Cervical cytology training and current update
  • Immunisation training and updates (childhood and adult)
  • Travel health experience
  • Experience using EMIS Web clinical system
