We are a dynamic and growing training practice serving a diverse patient population across three sites in West London. At CMP, we pride ourselves on access, continuity, and innovation our digitally enabled same-day access model and proactive care hub mean we are leading the way in modern general practice.
You will work as part of our large multidisciplinary clinical team, including GPs, Physician Associates, Clinical Pharmacists, Paramedics, HCAs, and the wider administrative and management team.
This role offers variety and autonomy in a supportive environment, with opportunities to develop specialist interests, participate in quality improvement, and contribute to the training of student nurses and other healthcare professionals.

As a Practice Nurse at Chiswick Medical Practice, you will provide high-quality, evidence-based nursing care to our registered population. Your duties will include (but are not limited to):
Delivery of routine and proactive nursing services, including cervical screening, childhood and adult immunisations, travel vaccinations, and wound care.
Management of long-term conditions such as asthma, COPD, diabetes, and hypertension (depending on experience).
Support for the proactive care team in monitoring and reviewing patients with complex needs or at risk of hospital admission.
Active participation in health promotion, screening, and prevention programmes.
Accurate record keeping using Systmone and adherence to clinical governance, infection control, and safeguarding policies.
Working collaboratively with the wider team to ensure seamless, patient-centred care.
Supporting the development of clinical protocols and contributing to audits and service improvement projects.
