Job summary

Brackley Medical Centre in Brackley is seeking an enthusiastic Practice Nurse to join our multidisciplinary team. You will deliver skilled nursing services to the patient population, focusing on health promotion and chronic disease management.

You will support the practice management team in reviewing and delivering clinical policies and procedures, and contribute to patient care across minor ailments and long-term condition clinics for Brackley and surrounding area.

Qualifications

• Experience as a practice nurse or primary care nurse.
• Ability to promote health and manage chronic conditions.
• Comfort delivering clinical policies and procedures.

Responsibilities

• Deliver nursing services to patients and promote health.
• Support chronic disease management and health prevention.
• Collaborate within a multidisciplinary team including GPs and nurses.

We'd love to hear from you!, Working as part of the practice multidisciplinary team, the post holder will ensure skilled nursing services are delivered effectively to the practice patient population. The practice nurse will be responsible for delivering health promotion, chronic disease management, health prevention, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedures.

We are GP teaching practice with a dedicated team committed to treat minor ailments, as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues for Brackley and the surrounding area. We have 5 partners, 6 salaried GPs in addition to 13 staff in the Nursing Team and Urgent Care Unit for a 16000 patient population.
