We are seeking an experienced and dedicated Practice Nurse to join our friendly and supportive multidisciplinary team. The successful candidate will play a key role in the delivery of high-quality nursing services within the practice, working collaboratively to meet the needs of our patient population.
- Deliver safe, effective, and evidence-based nursing care to patients within the scope of professional practice.
- Provide health promotion, screening, and disease prevention services.
- Support patients with the management of chronic diseases, including diabetes, asthma, COPD, and hypertension.
- Perform clinical tasks such as wound care, vaccinations, cervical screening, ECGs, and other routine procedures.
- Contribute to the development, review, and implementation of clinical policies, protocols, and procedures.
- Participate in quality improvement initiatives and audits.
- Work collaboratively with GPs, healthcare assistants, and other members of the multidisciplinary team to provide holistic patient care.
- Maintain accurate and timely clinical records in accordance with professional and legal standards.
- Develop, implement, and embed health promotion and well‑being programmes.
- Implement and evaluate individual treatment plans for patients with chronic diseases.
- Identify, manage, and support patients at risk of developing long‑term conditions.
- Provide routine nursing care in accordance with clinical evidence.
- Deliver travel medicine services, including vaccinations and advice.
- Request and process pathology services and results as required, ensuring appropriate follow‑up actions.
- Provide and manage chronic disease clinics.
- Maintain accurate and comprehensive clinical records.
- Ensure read codes are used effectively and maintain chronic disease registers accurately.
- Develop, implement, and embed Well Woman and Well Man clinics.
- Deliver opportunistic health promotion and lifestyle advice during patient consultations.
- Prioritise health issues and intervene appropriately to support early intervention and prevention.
- Implement vaccination programmes for both adults and children in line with national guidelines.
- Support patients in the safe and effective use of prescribed or over‑the‑counter medicines.
- Chaperone patients where appropriate.
- Recognise, assess, and refer patients presenting with mental health needs.
- Liaise with external services and agencies to ensure appropriate support for vulnerable patients and those with complex care needs.
- Support the clinical team with safeguarding matters.
- Empathetic communicators, able to engage effectively with patients, colleagues, and external agencies both face‑to‑face and over the phone.
- Self‑motivated and well‑organised, able to manage workload effectively and maintain high standards of care.
- Punctual and reliable, demonstrating excellent time‑keeping and commitment to the team.
