Job responsibilities

Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients. The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately. The post will be held at Riverside Surgery, 525 New Chester Road, Rock Ferry, Wirral CH42 2AG. On occasion there may be a requirement to undertake home visits.

Duties and responsibilities:

• Management of chronic disease
• Identifying significant abnormalities
• Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
• Advanced knowledge in the diagnosis, control and management of Asthma, COPD and Diabetes is essential
• Knowledge in Hypertension, CHD, AF, PAD, Stroke, CKD, Mental Health, Learning Disability, Depression, Rheumatoid Arthritis, Palliative Care and Epilepsy is desirable but not essential as training can be given

Qualifications

• 1st Level Registration in Adult Nursing
• Membership of a Professional Body
• Minimum of at least 2 years experience in Practice Nursing, in Primary Care
• Knowledge re EMIS
• Advanced knowledge in the diagnosis and management of Asthma, COPD, Diabetes and other chronic diseases
• Cervical Cytology Screening and Competency
• Immunisations and Vaccinations training for both Adult and Child
• Understanding of QOF
• Demonstrate an understanding of audits
• Good understanding of Health and Safety
• Good understanding of Infection Control

Experience

• RGN Registration in Adult Nursing
• Membership of a Professional Body
