Job description

Please note that under the PCN Network DES, the PCN can employ a practice nurse'except where they have held a post in the PCN, or any practice within the PCN, within the previous 12 months'.

We have an exciting opportunity for a Practice Nurse to join our friendly and experienced team, part of the Barnsley PCN. Reporting to the Lead Nurse and supported in practice by an ANP and/or GP, this role will deliver practice nursing services and care to the practice population at our Chapelfield site on a part time (22.5 hours per week) contract. They will provide care within the boundaries of their role as part of a multidisciplinary team, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. You will be joining a fantastic team of supportive colleagues.

Main duties

Main duties and tasks may include but are not limited to:

  • Cervical smears
  • Chronic Disease Management
  • COPD/Asthma
  • Removal of sutures
  • Urinalysis
  • Peak flow readings
  • Venepuncture
  • Blood pressure measurements
  • Ear Care
  • Routine childhood and adult immunisations
  • Requesting basic pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols with referral to GPs as appropriate
  • Administration of Depo injections, Zoladex, B12
  • Contraception Pill reviews
  • Leading Infection Control
  • Maintaining and cleaning equipment used by the nurses and GPs
  • Maintaining nurses rooms, stocking and rotating items as required
  • Chaperoning and assisting patients who are being examined by another clinician (when required)
  • Undertake housekeeping duties including general tidiness and cleanliness of nurses and treatment rooms
  • Ordering of stock
  • Participation in administrative systems in the practice including QOF/IIF/ PDA
  • At all times there will be a need to maintain accurate records
  • A duty to advise GPs of potential problems or errors within the range of assigned tasks
  • Attend and participate in our monthly governance meetings when required

They will ideally have experience within chronic disease management, COPD and Asthma. Experience in diabetes would be advantageous. The applicant must be self-motivated and adaptable to work within a changing NHS and to thrive with the demand of working in a busy GP practice.

The ideal candidate will have a commitment to maintain a high professional standard of nurse care and to keep up to date with all aspects of nursing care relevant to the post. It is essential that they have the experience of working in accordance with the NMC Code of Professional Conduct, working within a Primary Care Environment and experience of EMIS.
