Are you interested in being a part of an exciting initiative within the Specialist Learning Disability Service in Hertfordshire?

Do you have the passion and energy to make a real difference for people with an intellectual disability and their support network?

The Positive Partnerships Team (PPT) is part of the Specialist Learning Disabilities Service in Hertfordshire. PPT provides training and additional support to care provider organisations, working within the framework of Positive Behavioural Support and other person-centred approaches. We are currently looking to recruit a skilled and experienced Coordinator who will work alongside colleagues within PPT, and the wider Specialist Learning Disability Service, to help strengthen carers’ capacity to provide robust and skilled support for people with a learning disability living in a supported environment.

If you have the skills, qualification and experience of working within the Positive Behavioural Support framework and would like to apply your skills and knowledge to help others develop their approach, then this could be for you. The Positive Partnerships Team is an integral part of our Specialist Learning Disabilities Service and works closely with other health and social care services within Hertfordshire.

The Post will be based at Colne House, Watford. Travel by car throughout the area covered by the service is essential.

Responsibilities
  • Offer additional input for people with a learning disability to live in welcoming and supportive communities, working within the principles of PBS.
  • Provide person-centred, flexible and responsive support to assist individuals, support workers, carers, services and communities to access accurate and timely information to help clarify the person’s goals, strengths and needs, in accordance with their PBS Plan.
  • Support and contribute to the delivery of PPT’s training suite in PBS, Person Centred Active Support and other person centred approaches, and providing opportunities to work with support workers/carers to help embed their learning into the day to day, via Practice Development sessions.
  • Operate as a service coordinator, helping the referred person and their support worker/carers to plan, select and receive needed supports and services.
  • Assist people and their supports/services to manage barriers and constraints in achieving their preferred lifestyle.
  • Monitor the implementation of established PBS plans and protocols.
  • Support the development of inclusive communities through partnership and collaboration with individuals and support worker/carers, local organisations and the broader community.

PPT is an integral part of the Specialist Learning Disabilities Service in Hertfordshire.
