• Over 12 months experience at Band 5.
• Experience in managing patients at risk due to Diabetes.
• Evidence of CPD portfolio.
• Wound care experience.

Responsibilities

• Maintain high standards of clinical practice.
• Manage patient caseload efficiently.
• Develop foot-care treatment plans.
• Implement access criteria for treatment plans.
• Maintain accurate patient records.
• Participate in staff meetings and audits.
