Qualifications

• Essential: Experience with lower limb conditions and podiatry presentations; able to work independently and within a multidisciplinary team.

Responsibilities

• Assess, investigate, and manage foot conditions within a PCN multi-disciplinary team.
• Refer patients to appropriate services/direct pathways as needed.
• Manage orthotics, dressings, and equipment; contribute to developing podiatry services across the PCN.
• Provide education on foot health and clinical governance, including audits and CPD.
• Liaise with practice teams, community and secondary care staff to coordinate care.
• Support service development to improve patient care and pathways across Headingley, Horsforth, Burley, Kirkstall and Cookridge areas.

Woodsley & Holt PCN would like to recruit a Podiatrist to work within our Primary Care Network (PCN) multidisciplinary healthcare team. This role will be based across the PCN area of Headingley, Horsforth, Burley, Kirkstall and Cookridge areas.

Our ambition is to improve management of patients suffering with various foot conditions and other at-risk groups that have limited access to our community podiatry service.

Main duties of the job

This role will work closely with other PCN roles: Pharmacists, Physiotherapists, Dietician and Social Prescriber in addition to the core practice teams.

You will assess, investigate, manage foot conditions and refer patients who have been directed to your service by a primary healthcare worker or directly by reception where appropriate. You will be an experienced practitioner who is comfortable working independently as well as part of a wider multidisciplinary team. You will be supported by the clinical team at each GP surgery and work with the wider clinical team to deliver holistic care to our patients. You will be able to assess and assist in adapting the service in response to local population needs.

About us

Woodsley & Holt Park PCN is a large primary care network (PCN) with a combined patient population of approximately 100,000 across 10 GP practices, covering the areas of Woodhouse, Kirkstall, Burley, Hyde Park, Headingley, Cookridge and Horsforth. The PCN footprint is of mixed diversity with pockets of deprivation and health inequalities. As one of the largest PCNs in Leeds we work at pace to deliver high quality services to our patients, which will evolve with the continuing development of the PCN.

Job responsibilities

• Work as part of a PCNs multi-disciplinary team to clinically assess, treat, and manage a caseload of patients of all ages with lower limb conditions and footpathologies, using their expert knowledge of podiatry for specific conditions and topics
• Utilise and provide guidance to patients on equipment such as surgical instruments, dressings, treatment tables and orthotics
• Work towards developing the service to include the options to prescribe, produce, and fit orthotics and other aids and appliances
• Provide specialist treatment and support for high-risk patient groups such as the elderly and those with increased risk of amputation
• Work towards developing the service to include supporting patients through the use of therapeutic and surgical techniques to treat foot and lower leg issues (e.g. carrying out nail and soft tissue surgery using local anaesthetic)
• Deliver foot health education to patients
• Implement all aspects of effective clinical governance for their own practice, including undertaking regular audit and evaluation, supervision and training
• Liaise with PCN multi-disciplinary team, community and secondary care staff, and named clinicians to arrange further investigations and onward referrals
• Communicate outcomes and integrate findings into their own and wider service practice and pathway development
• Develop, implement and evaluate a seamless podiatry support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working.

Professional Responsibilities

• To maintain Registration with the Health and Care Professions Council (HCPC), and maintain professional standards of treatment and conduct
• Undertake continued professional development to understand the mechanics of the body in order to preserve, restore, and develop movement for patients;
• Provide education and specialist expertise to PCN staff, raising awareness of good practice in good foot health;
• Ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate PCN and local population objectives;
• To encourage multi-disciplinary working between all professional disciplines, developing a liaison culture.
• To ensure all duties are carried out to the highest standard and in accordance with current quality initiatives within local areas and to meet departmental activity targets.
• Actively participate in, and show personal commitment to, structured Continuing Professional Development [CPD] and a variety of learning opportunities outside structured CPD, in order to develop new skills, and maintain and update existing skills.
• To ensure high quality delivery of the Podiatry service by fulfilling CPD requirements and objectives identified through the appraisal process.
• To attend all Mandatory Training sessions as required by the PCN

Qualifications

Essential

• BSc or pre-reg MSc in Podiatry under a training programme approved by the College of Podiatry
• Registered member of the Health and Care Professionals Council (HCPC)
• Able to operate at an advanced level of practice
• An appreciation of the nature of GPs and general practice
• Experience of core indicative patient presentations including but not limited to:
• Skin and nail pathology
• Circulatory disorders affecting the lower limb
• Neurological disorders affecting the lower limb
• Biomechanics
• Rheumatology conditions affecting the lower limb
• Endocrine and metabolic disorders affecting the lower limb
• Care of the Elderly
• Podopaediatrics
• An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners Experience of working with multidisciplinary teams
• Involvement in working with and developing departmental protocols/procedures
• Knowledge and experienced in using System One and EMIS
