Job summary

Eden Brown Synergy is seeking a Social Worker to join the Hospital Discharge Team at Derriford Hospital, Plymouth. Hybrid working and competitive pay up to £35 per hour.

The role involves supporting discharges for people with complex social care needs and completing follow-up reviews in the community, within a MDT including nurses and health staff. You should be able to work independently and as part of a team.

Qualifications

• Must be Social Work England registered.
• Ability to work independently and in a team with strong analytical and communication skills.
• Ability to prioritise referrals based on risk assessments.
• Driver with access to own car and UK residency preferred.

Responsibilities

• Support hospital discharges for people with complex social care needs.
• Complete follow-up reviews within the community.
• Work within MDT with nurses and health professionals.
