What You Will Do

Deliver high-quality physiotherapy across all pathways of Intermediate Care, including reablement, rapid response, and rehab beds.

Work collaboratively within multidisciplinary teams to ensure effective, holistic patient care.

Utilise post-graduate experience to manage complex caseloads, providing tailored support for each patient’s individual needs.

Conduct detailed patient assessments and create personalised treatment plans to achieve the best rehabilitation outcomes.

Apply your community experience to deliver home-based interventions that promote independence and improve patients’ quality of life.

Requirements

Minimum of two years’ postgraduate experience, ideally within a multidisciplinary or rehabilitation setting.

A valid driving licence is advantageous for community visits.
