We're looking for a compassionate and skilled professional to join our team, supporting people in their journey to improved mobility, independence, and wellbeing.
In this role, you'll work with a range of conditions, from neurological conditions like stroke and Parkinson's to musculoskeletal issues such as arthritis and fractures. You'll also support patients recovering after hospital discharge, working closely with our Reablement team to encourage mobility and maximise their potential.
You'll assess, treat, and manage patients, tailoring treatment plans to their individual needs and adjusting them as they progress. Your ability to communicate complex clinical information in a clear, supportive way will be essential, as will your ability to work with patients facing communication or cognitive challenges. You'll also play a key role in safeguarding, ensuring the highest standards of care and professional practice.
Were looking for someone with a degree in physiotherapy, with at least 2 years post-qualifying experience, HCPC registration, and experience working with diverse and sometimes challenging client groups. You'll need excellent interpersonal skills, a proactive mindset, and the ability to travel to patients homes and other care settings.

Treatment plans are developed using clinical reasoning based on assessment outcomes and adjusted as needed to reflect progress. Informed consent is obtained for all assessments and treatments, ensuring customers understand their condition, rehabilitation potential, and options. Capacity to consent is assessed where cognitive or other impairments may affect decision-making.
The role requires ongoing monitoring and review of progress, adapting goals and plans to ensure effective outcomes. Compliance with local policies, procedures, and professional standards is critical, including safeguarding responsibilities.
A strong understanding of frameworks such as Protection of Vulnerable Adults and Wiltshire Council policy is required, with active participation in safeguarding investigations when concerns arise. This position demands a commitment to safe, effective, and person-centred care.

Job Purpose
Specific duties and responsibilities include
Provide comprehensive assessment, treatment, management, onward referral, and dischargefor a diverse range of customers referred to the service, ensuring each receives appropriateadvice and/or signposting for onward support.
Prioritise workload while managing the demands of customer needs, families, otherprofessional staff, and agencies, adapting to an increasing caseload complexity anddelivering the service in a way that suits the customers preferred communication method.
Maintain professional record-keeping standards, ensuring records are accurate, current, andaligned with professional codes as detailed by HCPC and Chartered Society ofPhysiotherapists (CSP), service standards, and organisational requirements.
Plan and implement treatment support plans based on assessment results, applying clinicalreasoning skills, and adjust these plans as needed based on customer progress.
Ensure informed consent for assessments and treatments, providing customers with a clearunderstanding of their condition and rehabilitation potential, and assess their capacity toconsent to treatment, with due consideration to insight where capacity is questioned orpotentially affected as result of cognitive or other impairment.
Monitor and review customer progress, adjusting support plans and treatment goals asneeded based on review outcomes to ensure the most effective approach is being utilised.
Comply with local policies, procedures, and professional standards, as outlined by the Healthand Care Professions Council. Customers may include vulnerable adults therefore develop aworking knowledge of relevant procedures including Protection of Vulnerable Adults and otherlegal frameworks.
Uphold safeguarding responsibilities, ensuring that adult safeguarding is carried out inaccordance with the law, local policy, and Wiltshire Council policy guidance, includingidentifying concerns and participating in safeguarding investigations.
Create a positive, caring environment where customers can expect high-quality, professionalservice, and where adherence to treatment and self-management is encouraged for optimaloutcomes.
Undertake complex manual handling techniques (with appropriate training), ensuring safetyand adherence to organisational policies for risk assessment and equipment use.
Order necessary equipment to support treatment and ensure it aligns with customer conditionand support plans.
Work in accordance with standards of practice set out by Chartered Society of Physiotherapy(CSP) and relevant national and local guidelines, ensuring treatment meets all professional andorganisational requirements.
Promote customer adherence to treatment and self-management, aiming for the best possibleoutcomes in line with customer goals.
Utilise manual therapy techniques, demonstrating skill in dexterity, coordination, palpation, andsensory assessment to enhance treatment effectiveness.
Address psychosocial factors in customer treatment plans, using appropriate techniques toconsider mental and emotional factors impacting customers recovery.
Be an ambassador for the council at all times, always representing the council positively,professionally and appropriately at meetings with customers, external partners and agencies.
Provide supervision and training as required by service to support development of lessexperienced team members and continued development of service.
Undertake and be responsible to own Continued Professional development to maintainregistration responsibilities outlined by HCP and CSP.
Specific requirements for this post
Ability to travel on a daily basis and to undertake visits to customers in their own homes or inother settings e.g. hospital.
There will be a requirement to occasionally work outside of standard hours, includingweekends to meet the needs of the service, for example assessing customers abilities atdifferent times of the day or provide support to other team members. To be flexible and workin other reablement teams as required.
