Job summary

Locala Health and Wellbeing invites applications for a Senior Physiotherapist to join the Urgent Community Response team. You will assess, treat, and evaluate patients referred during crisis periods and deliver short-term programmes to stabilise care in the community.

The role requires experience in community settings, strong communication, and the ability to travel to service sites. You will work with multidisciplinary teams to ensure patient-centred care and timely interventions.

• Contribute to the avoidance of unnecessary hospital admissions through timely and effective care.
• Manage a caseload of adults aged 18+ with complex health and social care needs in the community.
• Support individuals to achieve their maximum level of independence by improving physical and mental health outcomes, enhancing coping capacity, recommending appropriate equipment and interventions and working closely with multidisciplinary teams to ensure holistic, patient-centred care.

About the role

We are seeking an experienced and motivated Physiotherapist with a strong community background to join our dynamic and forward-thinking team. The ideal candidate will demonstrate a positive attitude, excellent communication skills, and the ability to work flexibly and collaboratively to meet the needs of the Urgent Community Response Team. You need to have the ability to travel independently to meet service needs and commitment to delivering high-quality, responsive care. This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.
