Leeds Community Healthcare NHS Trust are looking for a Band 5 Physiotherapist to join our Core Therapy or Active Recovery Neighbourhood teams. This role supports patient flow through rapid response/admission prevention work and facilitation of hospital discharges with a Home First approach. The service covers a wide range of clinical conditions including Falls reduction and prevention, rehabilitation and case management of long-term conditions such as Neuro, MSK, Orthopaedics, Respiratory, Rheumatology, Frailty and Palliative and end of life care. The post is based in the South hub area with citywide responsibilities and potential cross-cover requirements. Access to a car for work purposes is essential.
