Job description
Overview

Are you a newly registered Physiotherapist, starting your career? Leeds Community Healthcare NHS Trust are looking for a Band 5 Physiotherapist to join our Core Therapy or Active Recovery Neighbourhood teams. This role supports patient flow through rapid response/admission prevention work and facilitation of hospital discharges with a Home First approach. The service covers a wide range of clinical conditions including Falls reduction and prevention, rehabilitation and case management of long-term conditions such as Neuro, MSK, Orthopaedics, Respiratory, Rheumatology, Frailty and Palliative and end of life care. The post is based in the South hub area with citywide responsibilities and potential cross-cover requirements. Access to a car for work purposes is essential. Responsibilities
  • Deliver holistic rehabilitation for patients, either independently or by delegating tasks to others, as part of your own caseload within a multidisciplinary team.
  • Provide care across a 7-day service from 8am to 6pm, including lone working as required.
  • Participate in rapid response/admission prevention activities and support hospital discharge planning with a Home First approach.
  • Engage in regular clinical supervision, peer support and continuing professional development to support progression within the Trust.
  • Collaborate with the wider multidisciplinary team to enhance knowledge and patient care.
  • Be flexible to work across the South hub area with potential cross-cover within a citywide service, and consider caring responsibilities.
  • Demonstrate commitment to patient-centred care and effective management of a diverse caseload including patients with falls risk, long-term conditions, and palliative care needs.
Qualifications and Requirements
  • Newly registered Physiotherapist (Band 5) or eligible for registration.
  • Proven ability to work as part of a multidisciplinary team and to manage a personal caseload.
  • Access to a car for work purposes is essential.
  • Right to work in the UK (UK or Irish citizen, EU settlement scheme, or valid work visa).
  • Understanding of flexible working and ability to adapt to service needs.
