Job summary

Central London Community Healthcare NHS Trust is seeking a Senior Physiotherapist to join the Brent Urgent Community Response (UCR) Team. In this fast-paced, home-based role you will deliver specialist assessment, treatment and rehabilitation to people in their own homes, care homes and community settings to prevent hospital admission.

You will work within a multidisciplinary team to support recovery and promote independence.

Qualifications

• Registered Physiotherapist with HCPC registration.
• Experience in community or home-based settings is desirable.
• Ability to work autonomously within a multidisciplinary team.

Responsibilities

• Deliver holistic assessments and evidence-based physiotherapy interventions tailored to individual needs.
• Prescribe and advise on equipment and home adaptations to support safe care at home.
• Contribute to care planning, risk assessment and clinical decision-making.
• Support, supervise and develop junior colleagues and students.
• Build partnerships with patients, carers, families and multidisciplinary colleagues.

As a Senior Physiotherapist within our fast-paced service, you will provide specialist assessment, treatment and rehabilitation to individuals who may otherwise require hospital admission. Working directly with patients in their own environment, you will support recovery, promote independence and help them remain safely within their community. You will be part of a supportive multidisciplinary team responding rapidly to changing patient needs and delivering compassionate, person-centred care. Working closely with health, social care and community colleagues, you will contribute to integrated care plans and positive patient outcomes. As a Senior Physiotherapist within the Brent UCR team, you will deliver high-quality assessment, treatment and rehabilitation across community settings, including patients' homes, care homes and residential facilities. Working as part of a multidisciplinary team, you will support admission avoidance, facilitate timely hospital discharge and promote independence and recovery. Key responsibilities include:
