Job description

We're offering a fantastic opportunity to join our dynamic and supportive Intensive Rehabilitation Service (IRS) – a multidisciplinary community team that plays a vital role in reducing hospital admissions and supporting early discharges.

Responsibilities
  • Manage your own caseload of community-based patients
  • Deliver high-impact, person-centred rehabilitation in patients' homes
  • Collaborate with inpatient and acute teams to ensure seamless transitions of care
  • Provide supervision, training, and joint sessions with colleagues across the team
  • Deliver high‑quality rehabilitation in a patient’s home rather than a community bed setting, as part of a multi‑professional service/team
  • Work with patient-centred, goal‑orientated rehabilitation to promote self‑management and independence throughout the continuum of care
  • Provide onward referrals to other community services, including health and social care, to ensure safe management when completing IRS episode of care
  • Assess, manage, plan and deliver care as a Registered Physiotherapist (HCPC), being professionally and legally accountable
  • Support peers and team leader, be an effective team member while working independently and support collaborative working with other health and social care professionals and agencies

High Cost Area Supplement – Outer London: This post also attracts payment for a High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,714 and a maximum payment of £5,941 per annum, pro rata for part‑time).
