Are you passionate and motivated to use your clinical knowledge and assessment skills to support patient discharge from hospital and prevent admission? We are offering an opportunity to become part of the Urgent Community Response (UCR) team in Exeter South & West, working within a motivated multidisciplinary team to optimise people's abilities and quality of life and keep them as independent as possible.
We currently have a 12‑month secondment / fixed‑term opportunity for a motivated physiotherapist to join the UCR team in the Exeter South & West Cluster. The team offers a supportive and collaborative environment within an MDT, including OTs, physios, nursing, pharmacy clinicians and support worker colleagues.
For more detail about the main duties of this role, please see the job description section below. As a key member of the Royal Devon University Healthcare NHS Foundation Trust, you will be joining an exceptional team working together to help our patients stay healthy when they need it most.
A successful candidate should have a keen interest in rehabilitation and promoting independence of patients with a variety of long‑term conditions. The team supports the facilitation of hospital discharge as well as prevention of admission, working to optimise patients' ability and quality of life. The role involves close working with community rehabilitation and community nursing teams and is well supported by clinical leadership, a structured supervision and in‑service training programme.
The service operates from 8 am–8 pm, 7 days a week and covers the areas of Broadclyst, Pinhoe, Topsham, Exminster, Alphington, St Thomas and Starcross.
Working pattern: 37.5 hours per week, shifts on a rota basis. Please contact us to discuss working patterns further.
