Job summary

Burlington Primary Care in Ipswich is seeking a Physician Associate to join a growing MDT and develop a specialist LTC-focused caseload under GP supervision.

You will run structured chronic disease clinics, conduct medication reviews and care home visits, and contribute to proactive care planning within national QOF/IIF frameworks, while developing expertise in frailty and multi-morbidity management.

Qualifications

• Registered Physician Associate with GMC.
• Postgraduate Diploma or Master’s in Physician Associate Studies from a UK-accredited programme.
• Demonstrable interest or experience in long-term conditions management.

Responsibilities

• Run structured chronic disease clinics for patients with long-term conditions.
• Conduct medication reviews and annual reviews for multimorbidity.
• Undertake regular care home ward rounds and community visits.
• Support proactive care planning and prevention initiatives.
• Work with GP lead within a supervised model and MDT setting.
