Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits for our office located in Concord, NH, and surrounding areas. This office services adult and geriatric clients on a per-visit basis in territories throughout Grafton County, New Hampshire.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities for a PT include:

  1. Make home visits to clients in designated geographic territories.
  2. Perform diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
  3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  4. Continuously assess and revise care plans, collaborating with nursing staff as appropriate.
  5. Educate clients and their families on rehabilitative care and activities to promote health, safety, and independence.
  6. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications:

  • Current New Hampshire Physical Therapist (PT) license.
  • At least one year of recent work experience as a PT.
  • Graduation from an American Physical Therapy Association-approved program, evidenced by transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
