Job summary

A healthcare recruiting firm is seeking a Physical Health Monitoring Team Leader for the Northern Ireland area. Job description
Physical Health Monitoring Team Leader Band 7

The post holder will be responsible for assisting with the development of the Physical Health Service, delivering skilled evidence based interventions to service users and carers/families with challenging complex needs and/or who present high levels of risk. The post holder will be responsible for the ongoing development of the physical health pathway and monitor the physical health of service users open to the team in line with the NICE guidelines. The post holder will assist in supervision and will supervise and develop members of lower band staff and will provide mentorship to students. The post holder will have a professional leadership responsibility for nursing staff within physical health team, including revalidation, updating EIMS mentorship and providing support to mentors.

Responsibilities
  • To manage the defined caseload, Undertake physical health assessment of individuals’ within the mental health service.
  • Plan and evaluate and review service user needs co-ordinating and reviewing care needs when required.
  • Where appropriate, to assess the needs of clients referred under Trust Care Management arrangements in consultation with clients, carers and other professionals
Skills and Qualifications
  • Have two years continuous experience at Band 6 or above in an adult mental health setting.
  • Experience in physical health monitoring.
  • Proficient in venepuncture.
