  • Maximise safe, effective and cost-efficient use of medicines.
  • Provide support on medicine related issues to the whole practice team.
  • Provide support with medication reconciliations.
  • Provide support with prescribing audits in line with CQC & QOF targets.
  • Be a point of contact for patients regarding medicines queries.
  • Work with primary care team and patients to resolve medication queries.
  • Assisting with repeat prescribing and ensuring safe dispensing.
  • Monitoring adherence to practice, local and national prescribing policies.
  • Supporting clinical pharmacists in delivering clinical pharmacy services.
  • Providing advice and information to patients and other healthcare professionals.
  • Undertake structured medication reviews.
  • Triaging and managing common ailments.

Job description

The PCN Pharmacy Technician will work alongside another Pharmacy Techncian and under thesupervision of a Senior Clinical Pharmacist and as an integrated part of the PCNClinical Pharmacy Team to ensure the safe, accurate and timely supply ofprescribed medication to patients. The role will contribute to successfulclinical management of patients and their needs within the GP practicesincluded in Synergy Health PCN.

SynergyHealth PCN covers three practices in Gedling Borough, Jubilee Park MedicalPartnership in Carlton with a branch site in Lowdham, West Oak on WestdaleLane, Mapperley and Trentside Medical Group in Netherfield and Carlton.

Weare a standalone PCN with a lead practice. We have a population of 36,000patients.

ThePCN has care co-ordinators, social prescribing link workers, advanced nursepractitioner, PCN GPs and clinical pharmacists. These roles will work closelytogether to develop and grow the PCN and improve patient care across ourpatient list.

Ourvision is to build and develop collaborative working across member practicesand stakeholders, to support and strengthen the delivery of Primary Careservices and improve health outcomes for the local population we serve.
