Job Summary

We have an exciting opportunity for a new role for an experienced and highly motivated individual to join the One Life Primary Care Network (PCN) team in the town of Hartlepool. We are looking for a highly motivated Pharmacist Technician to work within our established Pharmacist team. The successful candidate will be organised, self‑motivated, keen to learn and develop, hardworking with a good team ethic, along with excellent written and verbal communication skills.

Main Duties
  • Under the management of the Clinical Pharmacist deliver a comprehensive medicines management hub in a primary care setting and ensure a safe and efficient service is maintained, delivering a range of medicines management activities including medicines reconciliation and medicines history taking.
  • Work across the One Life Hartlepool Primary Care Network, supporting all three Practices.
  • Liaise with pharmacy staff in all sectors and other healthcare professionals to ensure a patient‑oriented medicines management service is provided.
  • Work closely with the Clinical Pharmacists to adopt a team approach to medicines management, and support healthcare professionals and the wider team with medication queries.
  • Provide support to the Clinical Pharmacist on medicines management audits.
  • Play a pivotal role in improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice.

Three friendly GP Practices, working within the same building offer this exciting opportunity for a brand‑new position within the One Life Hartlepool Primary Care Network. Our forward‑thinking Practices have 34,000 patients and a range of Clinical Disciplines supporting the GP team, including Clinical Pharmacists, Social Prescribing Link Workers, Mental Health Practitioner, First Contact Practitioner and Care Co‑ordinator. Our PCN supports several training disciplines, including GP Registrars and Medical students and can offer a strong mentoring and training approach.

Job Responsibilities
  • Support Practice Clinical Pharmacists and Practice teams to provide a safe and effective repeat prescribing service for patients.
  • Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and managing these changes within their own level of competence.
  • Provide primary support to general practice staff with regard to prescription and medication queries.
  • Support areas of prescribing and medicines optimisation management by working directly with patients, carers and support groups.
  • In collaboration with the Clinical Pharmacists and GP Prescribing Leads identify national and local policy and guidance that affects patient safety through the use of medicines, including MHRA alerts, product withdrawals and emerging evidence from clinical trials.
  • Facilitate clinical audits and improvement projects or work with colleagues such as GP Registrars, Practice Managers, etc.
Qualifications
  • Essential:
    • Mandatory registration with General Pharmaceutical Society
    • NVQ Level 3 in Pharmacy Services or equivalent
    • BTEC in Pharmaceutical Science or equivalent
  • Desirable:
    • Membership of Association of Pharmacy Technicians UK (APTUK)
Experience
  • Essential:
    • Previous relevant experience working in Pharmacy
    • An in-depth knowledge of medicines management within a clinical setting especially medicines reconciliation and history taking
    • Experience of regular contact with patients/customers
    • Awareness of systems to support management of patients in a primary care setting, delivering pharmaceutical input
    • Good clinical pharmacy knowledge including terminology
    • Ability to organise and prioritise workload in order to meet deadlines and targets
    • Excellent IT Skills
    • Ability to share own knowledge, skills and experience
    • Good interpersonal skills
    • Good written and oral communication skills, including record keeping
  • Desirable:
    • Experience of participating in clinical audit
    • Experience of working within a multi‑disciplinary team
    • Knowledge and understanding of SystmOne
    • Good clinical, pharmaceutical and health promotion knowledge
    • Understanding of GMS Contracts and Medicines Management Incentive Schemes
