Threefriendly GP Practices, working within the same building offer this excitingopportunity for a brand-new position within the One Life Hartlepool PrimaryCare Network. Our forward-thinkingPractices have 34,000 patients and have a range of Clinical Disciplinessupporting the GP team, including Clinical Pharmacists, Social Prescribing LinkWorkers, Mental Health Practitioner, First Contact Practitioner and CareCo-Ordinator. Our PCN supports several training disciplines, including GPRegistrars and Medical students and can offer a strong mentoring and trainingapproach.
Support PracticeClinical Pharmacists and Practice teams to provide a safe and effect repeatprescribing service for patients.
To reconcile medicines following discharge fromhospitals, intermediate care and into care homes, including identifying andrectifying unexplained changes and manage these changes within their own levelof competence.
Provide primary support togeneral practice staff with regards to prescription and medication queries.
Support areas ofprescribing and medicines optimisation management by working directly withpatients, carers and support groups.
In collaboration withthe Clinical Pharmacists, and GP Prescribing Leads identify national and localpolicy and guidance that affects patient safety through the use of medicines,including MHRA alerts, product withdrawals and emerging evidence form clinicaltrials.
Facilitate clinical audits and improvementprojects or work with colleagues such as GP Registrars, Practice Managersetc.
