Job description
Pharmacist (Primary Care) - St Paul's Clinic

An exciting new opportunity for a Pharmacist to join our friendly practice team.

Face to face contact with patients and treating minor ailments.

You will maintain and improve quality general medical services and the management of prescribed medication for the registered patients of the Practice In collaboration with other clinical and managerial colleagues

You will also work towards the achievement of the clinical elements of the GMS contract and prescribing indicators.

Main duties of the job

As a Practice Pharmacist and Independent Prescriber you will deliver a daily Minor Ailments service, supported by the clinical team.

You will work within your scope of practice with the freedom to autonomously manage patient care. You will identify, assess and diagnose patients with complex health needs across the practice.

You will undertake patient medication reviews within the surgery, prescribe and update medications

As a training practice you will also help support with training and mentoring of medical students. You will have ongoing opportunities for continued professional development, mentorship and support, to ensure that you are competent and confident in your role.

In addition you will be directly responsible for leading several clinical areas including audits and chronic diseases

Job responsibilities

Reviews patients current medication and assess needs for acute medication

Makes critical judgements to satisfy the expectations and demands of the job eg first contact with patients presenting acutely and manages care in the presence and absence of protocols.

Advises and supports others where standard protocols do not apply.

Works within professional guidelines and codes of conduct.

Works within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.

Extended and supplementary prescriber.

Contributes to the strategic development of primary care and public health services to meet patient and population needs within the practice and across the wider organisation. Works and liaises across multi-professional and agency boundaries.

Works at the forefront of practice innovation. Determines how to deal with ambiguous or unique problems to develop and advance primary care services for the benefit of patients, carers and communities.

Recognition and involvement in St. Pauls Clinic as a teaching unit and help drive the pharmacy element of training, including supporting trainee independent prescribers

Involvement pilot services and other ventures in conjunction with St. Pauls Clinic.

Develops a special interest - dependent on experience, within the practice e.g. dementia assessments, learning disability reviews, diabetes management etc

Encourage and motivate others to be forward thinking in ideas that address the health needs of the practice that will be innovative whilst managing and developing new ways of working

Develops and leads on evidence based practice.

Promotes effective communication and relationships within the team.

Actively participates in clinical and practice meetings.

Develops own knowledge and practice to meet objectives/changes in service, through attendance on study days, self-directed learning and reflection on practice.

Assists in the upskilling of trainee independent practitioners
