Band 8a Clinical Pharmacist – Mersey and West Lancashire NHS Foundation Trust & Liverpool Womens NHS FT

Location: Southport and Formby District General Hospital

Salary: Band 8a

Contract Type: Full‑Time (37.5 hours per week)

Overview

Mersey and West Lancashire NHS Foundation Trust is seeking a highly motivated and experienced Band 8a Clinical Pharmacist to join our dedicated team at Southport and Formby District General Hospital. This role offers a fantastic opportunity for an enthusiastic pharmacist to play a key role in delivering high‑quality clinical pharmacy services and optimising medication management for inpatients.

Responsibilities
  • Taking drug histories on admission and completing medicines reconciliation for inpatients.
  • Screening newly admitted patients for medicines optimisation issues and developing individualised care plans.
  • Conducting medication reviews to ensure safe, evidence‑based, and cost‑effective prescribing.
  • Monitoring and adjusting patient medication based on therapeutic drug monitoring results and response to treatment.
  • Counselling patients on their medication, ensuring they understand their drug therapy and any potential side effects.
  • Providing professional and legal clinical supervision in the dispensary, ensuring best practices in dispensing and medicines management.
Qualifications
Essential Criteria
  • Master's degree in pharmacy (MPharm) or equivalent qualification.
  • Registration with the General Pharmaceutical Council (GPhC).
  • Evidence of ongoing Continuous Professional Development (CPD)/Revalidation in line with professional requirements.
  • Previous relevant hospital experience in a clinical setting.
  • Strong foundation in Clinical Pharmacy and Medicines Optimisation.
  • Excellent verbal and written communication skills to liaise effectively with multidisciplinary teams.
  • Strong time‑management, decision‑making, and organisational skills.
  • Computer literacy and experience using clinical systems and electronic prescribing tools.
Desirable Criteria
  • Postgraduate qualification in Clinical Pharmacy or independent prescribing qualification.
  • Experience in a specialist clinical area such as acute medicine, critical care, or cardiology.
  • Previous experience supervising and mentoring junior pharmacists or pharmacy technicians.
  • Familiarity with NHS policies and procedures in medicines management.
