93 yr old male

He is severely sight impaired, needs support with meals but is able to eat on his own. Needs assistance with his medication during the morning. He walks the stairs and goes to bed unaided.

She has a diagnosis of vascular dementia (mid stage) and has restricted mobility. She relies heavily on the zimmer frame. Sometimes she forgets to use it so needs prompting to do so. She suffers from scoliosis on the spine and her back is crooked which pains her. Currently supported by a small team of PA’s who support with all daily living tasks. Needs support with meals but is able to eat on her own. She enjoys helping the PA with the baking and looks forward to sharing a cup of tea and a chat with them on their arrival – followed by a jigsaw or a puzzle.
