Job summary

A dedicated agency for social care roles is seeking a Personal Advisor for the Telford and Wrekin area. This hybrid role requires experience working with care experienced young people. You will undertake statutory responsibilities and support young people aged 16 to 25 as they transition to independent living. The ideal candidate will have strong skills in guidance and case management. An attractive pay rate of £21.25 per hour is offered.

Personal Advisor – Leaving Care – Telford and Wrekin

Pay rate £21.25 per hour

Contract role, Personal Advisor.

Experience within a similar role is essential.

Responsibilities

• Undertake the statutory role of Personal Adviser role for individual Care experienced young people from age 16 up to the age 21 or 25.
• To be the key professional and case manager to Former Relevant care experienced young people and when needed to those young people who are Relevant or Qualifying for advice and assistance.
• To work with young people leaving care and their support network to support the development of independent living skills and ensure a smooth transition to independence.
• To assist and support young people to prepare for leaving care and living independently; providing support, advice and guidance to them up to age 25.

There is plenty of hybrid working available.
