Location: Trinity Road, Bootle, L20 3NJ

Start Date: ASAP

Contract Duration: 4+ Month

Working Hours: Mon - Fri, 09:00 - 17:00, 37 Hours per week

Pay Rate: £ 19.00 per hour

Job Responsibilities
  • Support young people leaving or who have left care.
  • Offer advice and practical help.
  • Participate in creating and reviewing Pathway Plans.
  • Coordinate support services for young people.
  • Keep track of young people's progress and well-being.
  • Help young people access health services and improve their well-being.
  • Encourage young people to reach their potential.
  • Assess and report risks and safeguarding issues.
  • Work with various agencies to support young people.
  • Attend meetings to represent the department.
  • Maintain accurate records.
  • Ensure proper authorization and recording of case expenditures.
  • Contribute to service development through meetings and events.
  • Engage in performance reviews and training opportunities.
  • Perform other duties as needed.
Person Specifications
Must Have:
  • Experience in social care or related field.
  • Ability to work with young people and families.
  • Strong communication and organizational skills.
  • Understanding of safeguarding and risk assessment.
Nice to Have:
  • Experience with electronic record-keeping systems.
  • Knowledge of local community and faith groups.
