Job description

The post holder will be responsible for: Reviewing the needs of all service users in Nursing and Residnetial Care Home facilities identified by the trust on a systematic basis to ensure the appropriateness of the service, at all times seeking the views of service users, carers and the independent care providers where appropriate. The post holder will work under the supervision of the Team Leader and will have access to professional nursing supervision and support. They will be professionally accountable to the Lead Nurse for the Permanent Placement Team.

Responsibilities
  • To implement relevant legislation, Department and Trust Policies and Procedure.
  • Support the identified care homes in meeting the assessed need of the individual residents through staff knowledge and skills.
  • Functioning as a named worker supporting service users in permanent care and their carers through the care management process.
  • Assess the nursing care needs of service users who self-fund their nursing home care to determine their eligibility for the nursing care payment under the HPSS Payments for Nursing Care Scheme.
  • Liaising with relevant professional colleagues and providers within the statutory, voluntary and private sectors.
  • Managing an allocated workload and establishing priorities within it in the interest of good time management and delivery of an effective and efficient service.
  • The post holder will be an integral member of the Permanent Placement Team working in a complex multi-disciplinary environment.
Skills and Qualifications
  • Registered Nurse on Level 1, Part 1 of the live NMC Register. Adult nursing.
