Meeveem Limited is seeking a General Practitioner to join our Community Care Hall Green PCN team in Birmingham. The GP will work alongside practice and PCN staff, providing autonomous clinical care across surgeries, telephone consultations and home visits.

You will be professionally accountable under GMC, manage a diverse caseload across three practices, and contribute to high standards of patient care within the PCN. Experience with NHS guidelines and QOF will be advantageous.

Job summary

An exciting opportunity has arisen to join our Community Care Hall Green PCN team. We are a dynamic, and forward thinking PCN, who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

We are looking to expand our PCN team by seeking to employ a General Practitioner to work alongside our practices and PCN staff. As well as general practice staff, our PCN staff group currently also includes Clinical Pharmacists, Physicians Associates, Social Prescribers, Physiotherapists, Mental Health Practitioners, Care Coordinators, Nurse Associates and Pharmacist Technicians.

Main duties of the job

To perform as an autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting.

Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct, ensuring the safety of patients and the quality of their care is the main focus.

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients across a Primary Care Network of 3 GP Practices, dealing with a diverse range of health needs. The post holder will be based at Northbrook Group Practice &/or Swanswell Medical Centre.

This will include, but is not limited to, surgery and telephone consultations, on call, home visits, administration, complying with the practices contracts (GMS/PMS/enhanced services/DES).

Job responsibilities
Clinical Responsibilities are as below

In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion

Triage patients presenting to the surgery by telephone, online and in person when on call

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Assessing the health care needs of patients with undifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signs of illness

In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education

Manage long-term conditions and patients with complex problems

Recording clear and contemporaneous consultation notes to agreed standards

Collecting data for audit purposes and complying with QOF requirements

Delivering enhanced services

Check, manage & process patient test results

Refer patients to secondary care and/or relevant organisations and internally within the PCN (e.g. to additional roles members of the PCN team) as required

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) in line with local and national guidelines

Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate

In general, the post-holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basis
