Job Summary

We are currently seeking a dedicated and experienced Practice Nurse to join our busy Primary Care Network working across 4 practices.

This is a new role within the PCN – working in a hub and practice-based model. As a Practice Nurse, you will provide high‑quality nursing care to our patients and support the ARRS Roles in the PCN as well as the wider practice teams.

Responsibilities
  • Conduct health assessments, including physical examinations and health screening.
  • Provide health education and advice to patients on a range of topics, such as healthy living and disease prevention.
  • Administer vaccinations and other injections.
  • Take blood samples and carry out other diagnostic tests.
  • Support patients with long‑term conditions, such as diabetes and asthma.
  • Support practices with chronic disease management.
  • Perform cervical smears.
Qualifications
  • Registered Nurse with a valid Nursing and Midwifery Council (NMC) pin.
  • Minimum of 2 years' experience working as a Practice Nurse.
  • Experience in chronic disease management.
  • Experience in cervical smears.
  • Strong communication and interpersonal skills.

About Us

You will be joining a Primary Care Network of 4 member practices in the Hanley, Bucknall & Bentilee area with around 35,000 patients.

Based out of the PCN hub, you will either visit housebound patients or work from one of the member practices in clinics. We have a multidisciplinary workforce covering social prescribing, pharmacy, physiotherapy, mental health, paramedics and frailty.

Person Specification

Essential:

  • Up‑to‑date training in childhood immunisations, health promotion and clinical systems.
  • 2‑year post‑registration at E grade.
  • Family planning, smoking cessation and IT skills (MS Office, Internet).
  • Excellent interpersonal, time‑management, self‑motivation and confidentiality skills.
  • Knowledge of clinical governance and audits.

Desirable:

  • Accredited training in chronic disease management and anticoagulation management.
  • Cervical cytology qualification.
  • Evidence of recent updating in primary care and knowledge of clinical audits.
