As thePersonalised Care Team Lead, you will oversee and support the SocialPrescribing Link Workers within Rockingham Forest PCN. This role is aboutempowering patients to take control of their health and wellbeing. Our LinkWorkers help people with a wide range of needswhether they are managinglong-term conditions, experiencing low mood or loneliness, or dealing withcomplex social challenges.
You will beresponsible for the day-to-day supervision of the team, providing guidancethrough the development and review of processes, setting clear criteria, andstrengthening connections with local community and voluntary groups.
Alongside yourleadership responsibilities, you will also work directly within a personalisedcare roleeither as a Social Prescribing Link Worker, Health and WellbeingCoach, or Care Coordinator. Working as part of our multidisciplinary team, youwill support individuals to access non-medical services, build theirconfidence, and connect with community resources that address the wider factorsaffecting their health and wellbeing.
As the Personalised Care Team Lead, you will play a key role in shaping and supporting personalised care across the PCN. Youll lead a dedicated team of Social Prescribing Link Workers, Health & Wellbeing Coaches, and Care Coordinators, providing day-to-day supervision, guidance, and opportunities for staff development.
Working closely with GP practices, community partners, and voluntary organisations, youll help patients access the right support at the right time whether thats connecting with local services, building confidence to manage their health, or addressing wider social challenges.
Alongside your leadership role, youll also work directly with patients in a personalised care capacity, ensuring you remain connected to the people and communities you serve. By collaborating with the Digital & Transformation Manager, youll drive new initiatives, refine existing processes, and expand the support available, helping patients to live healthier, more independent lives.

Rockingham Forest PCN is a large, forward-thinking Primary Care Network compromising 3 practices across Corby, Kettering and Oundle covering a population of c.67k patients
This role is based across the Lakeside surgeries in Corby, Kettering and Oundle.

Primary Care Networks PCN are a group of local General Practice surgeries working in collaboration with one another to provide services to its local communities and patient groups. Lakeside Healthcare Group is responsible for 4 PCNs across 3 Localities and providing services to approximately 200,000 patients across these networks.
The Personalised Care roles Social Prescribing Link Workers, Health & Wellbeing Coaches and Care Coordinators play an important part in providing extra time, capacity, and expertise to support patients to be actively involved in managing their health and to make choices that are right for them. Working to the What Matters To Me model, personalised care delivered is focused on the strengths and needs of the individual.
The successful candidate will be responsible for leading and providing the day-to-day supervision and support for Personalised Care Team within the PCN; evolving and developing business as usual processes and working with the Digital & Transformation Manager to develop new initiatives to optimise the care options available for the PCN patients and communities. A first line of contact and guidance for site leads on the introduction and induction of new personalised care roles into the PCN.
