A leading healthcare provider in Maidstone is seeking a motivated Paramedic Practitioner. The role involves providing clinical assessments, conducting home visits, and managing care for a diverse patient demographic. Candidates should possess a Diploma in Paramedic Science, and a clean UK Driving Licence, with at least 2 years of relevant experience necessary. Be an experienced clinician, acting within professional boundaries to provide clinical assessment and management of patients within practice. Support the delivery of high-quality care and the development of innovative, evidence-based practice. Work as an autonomous, accountable Paramedic Practitioner providing holistic assessment, diagnosis and treatment. Provide and maintain a high standard of patient care while supporting GPs and the wider healthcare team.
Conduct home visits as required to assess both acute illness and long-term conditions.
Management of our housebound and frail patients, contributing to their care and carrying out assessments where appropriate.
Undertake in-practice clinical sessions for both triage and long-term condition management.
Work within national and local clinical guidelines and protocols.
Contribute to emergency response situations and support same-day care demand.
Maintain accurate and high standards of clinical documentation and coding in patient records.
Participate in clinical audits, meetings, and quality improvement initiatives.

Our PCN comprises four practices in the Maidstone area: The Medical Centre, Bower Mount Medical Practice, The Vine Medical Practice and Brewer Street Surgery.
Maidstone Central PCN is seeking to recruit a highly motivated and experienced Paramedic to work as part of a supportive team, delivering high-quality, patient-centred clinical services and helping to ease the demand on GPs. The role will require you to undertake a mixture of home visits and clinical sessions across the Maidstone area.

Assess and triage patients, including same day triage, providing definitive treatment or referring to other members of the primary care team.
Advise patients on general healthcare and promote self-management, including signposting to community or voluntary services.
Perform specialist health checks and reviews.
Perform and interpret ECGs.
Support chronic disease management (e.g. asthma, COPD, hypertension) under supervision or as part of a team.
Support the delivery of anticipatory care plans.
Lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients in sheltered housing).
Provide an alternative model to urgent and same-day home visits.
Communicate effectively across organisations to ensure patient-centred care.
Proactively engage with colleagues across the multi-disciplinary team, attending and contributing to meetings.
Commit to continuous professional development, sharing knowledge and behaviours to meet service needs.
Act within the Code of Conduct for your role and in line with organisational policies.
Maintain CPD and participate in annual appraisal with Line Manager.
Contribute positively to the effectiveness and efficiency of the service.
Understand own role and scope within the organisation and how it may develop.

Understand and utilise referral mechanisms within the practice.
